Thank you for your inquiry regarding CMS871v3 Hospital Harm – Severe Hyperglycemia. The statement that you reference from the guidance section of the measure is to clarify eligibility of Day 1 for Measure Observation 2. This portion of the logic did not change.
In your example encounter, eligible denominator days = 4 and eligible hyperglycemic event days = 1. DayNumber 1 is not a Hyperglycemic Event Day because the glucose of 400 mg/dL on 4/10 at 2:47am occurs in the first 24 hours of the encounter. DayNumber 3 is a Hyperglycemic Event Day because it is a day where a glucose was not measured, and it was immediately preceded by two contiguous, consecutive days where at least one glucose value during each of the two days was >=200 mg/dL.

      Hi Team,

Consider the following statement from guidance section of measure CMS871v3.

Although the measure does not count any hyperglycemic events that occur in the first 24 hours as a hyperglycemic event day in Measure Observation 2, the first 24 hours of the encounter is considered day 1. This is because if there was a day during the encounter where a glucose result is not found, the measure evaluates the two days preceding to see if each had a glucose value >=200 mg/dL. The measure allows the first 24 hours of the encounter, i.e., day 1, to be one of the preceding days.

      #Query:

As per the above statement we anticipate that there is slight change in measure observation calculation. However, this change is not mentioned in measure observation section of the document. So, do we need to consider this change in measure logic of CMS871v3?

If we need to do the logic change then consider the attached scenario and help us understand the number of eligible denominator days and hyperglycemic event days?

Kindly assist us on the above queries urgently as client deliverables are dependent upon it.
