Thank you for your question on Safe Use of Opioids - Concurrent Prescribing (CMS506v5). The measure follows the CDC Clinical Practice Guideline for Prescribing Opioids for Pain — United States, 2022, which does not have an exclusion for post-op or short term opioids. The measure intent is to encourage clinicians to carefully evaluate the need for concurrent medications. We recognize that there are some clinically appropriate situations for concurrent opioid and benzodiazepine prescriptions, and do not expect hospitals to have a score of zero. Please let us know if you have additional questions.

    • Increasing amount of patients in numerator for a limit timeframe prescription for pain management for post op surgery.

Our facility has many patients falling into the numerator due to having a surgery with being prescribed at discharge a PRN opioid for a short timeframe to help manage post-op pain. These patients also have home medications which include a long-standing benzodiazepine for psychiatry maintenance. Are these patients being incorrectly captured and/or should there be an 'exception and/or exclusion' for Safe Use of Opioids-Concurrent Prescribing?
