In order to meet the Numerator 2 criteria, documentation of "Intervention, Performed" is required using codes from the value set "Counseling for Nutrition" (2.16.840.1.113883.3.464.1003.195.12.1003). This value set includes SNOMED code 11816003 ("Diet education (procedure)"). Services that are clinically equivalent to the codes in the "Counseling for Nutrition" value set can be mapped to satisfy the requirements of Numerator 2 for CMS155v11.

      Please note that written self-help materials (e.g., brochures, pamphlets, discharge instructions) and complementary/alternative therapies are not included in the value set and do not qualify for the numerator on their own. Education pamphlets will only qualify if they are provided in addition to the nutrition counseling.

      According to the value set for "Counseling for Nutrition", organizations are allowed to map "Diet education (procedure)" (11816003). Would attaching discharge instructions to the patient's electronic health record that explain a particular diet/guide satisfy (i.e. Stoplight, Go-Slow-Whoa, etc.)? Or, does there need to be a component that addresses actual counseling in order to justify compliance?
