      As you noted, to document that a follow-up intervention has been performed, documentation of “Intervention, Performed” is required using codes from either the value set “Follow Up for Above Normal BMI” (2.16.840.1.113883.3.600.1.1525) or “Follow Up for Below Normal BMI” (2.16.840.1.113883.3.600.1.1528) based on the patient’s BMI. These value sets include SNOMED code 41077006 (“Special diet education (procedure)”). Services that are clinically equivalent to the codes in the “Follow Up for Above Normal BMI” or “Follow Up for Below Normal BMI” value sets can be mapped to satisfy the follow-up requirements for CMS69v12. Written self-help materials such as discharge instructions are not included in the value sets and will only qualify if they are provided in addition to a qualifying follow-up intervention.

      According to the value set for "Follow Up for Above Normal BMI", organizations are allowed to map "Special diet education (procedure)" (410177006). Would attaching discharge instructions to the patient's electronic health record that explain a particular diet satisfy (i.e. Mediterranean Diet, DASH diet, etc.)? Or, does there need to be a component that addresses actual counseling in order to justify compliance?
