      Thank you for your question on CMS71, Anticoagulation Therapy for Atrial Fibrillation/Flutter (STK-3). We apologize for the confusion regarding the the additional dose values of Enoxaparin. These values were not added to the anticoagulant therapy value set for stroke 3. They were added to the value set for antithrombotic therapy value set stroke 2&5, as you indicated. The RX Norm values sets use Semantic Clinical Drug (SCD) codes and are generalizable drug concepts and each RxNorm clinical drug name reflects the active ingredients, strengths, and dose form comprising that drug. The value set exclusion criteria for anticoagulant therapy "Excludes concepts that represent enoxaparin generally given for VTE prophylaxis."

      On CQM 5805 you indicate the Enoxapain 30mg and 40mg was added to the Anticoagulent Therapy oid 2.15.840.1.113883.3.117.1.7.1.200 value set for 2022 forward. We do not see this on that value set. We do see these in the Antithrombotic Value Set for Stoke 2 and Stoke 5.

      Please clarify whether whether the Enoxaparin is supposed to be on the Anticoagulant Value Set for Stroke 3? IF not, should we map this to the 854255 enoxaparin sodium 100 mg/ml injectable?
