      ​Thank you for your inquiry specific to CMS149v11 (2023). As currently constructed, the measure is a patient-based proportion eCQM that assess the percentage of patients, regardless of age, with a diagnosis of dementia for whom an assessment of cognition is performed and the results reviewed at least once within a 12-month period. As such, the initial population criteria includes an ‘Encounter to Assess Cognition’ with ‘Dementia’. Please follow the logic in the measure specification. The logic does not allow to evaluate attribution on a visit diagnosis.

      as mapping, our inflated population/denominator will affect our outcome since providers such neurosurgeons/ophthalmologist/pain medicine are seeing patients for a different condition that their dementia.

       We are a multispecialty group. CMS 149 will be a new measure to track; however, while checking the definition mapping by CMS it takes in consideration Encounter diagnosis or Problem diagnosis. The issue we have encountered is that our ophthalmologist, pain management and neurosurgeons who are not treating these patients for Dementia are going to impact our measure since cognitive assessment are not part of their current practice but since some of their patients may have dementia as a problem but not the reason to visit with some of our providers that are not neurologist they are being counted as denominator. Is there an option or can an evaluation on the mapping can be done to only include Visit Diagnosis?
