      The measure logic checks whether the Atrial Fibrillation/Flutter diagnosis occurred on or before the Ischemic Stroke encounter (snapshot of the measure logic below).

      As the scenario you have cited has a history of Atrial Fibrillation the case should meet the criteria of the denominator even if it is not documented in the current encounter.

      Please provide guidance on how to evaluate the following scenario:

      The patient was diagnosed with Atrial Fibrillation in the past (ex. a visit a year ago in the outpatient setting). The patient is seen in the hospital (current time), and the visit is coded with the principal diagnosis of Ischemic stroke. There is no documentation of Atrial Fibrillation in the current visit.

      Should the current Ischemic Stroke hospitalization (without Atrial Fib documentation in the current visit) be included in the STK-3 denominator? Thank you for your guidance.
