Our facility IT person who is currently building this measure is asking for risk factors related to the POA value set listed below. Are there separate risk factors vs ICD 10 code descriptions, or both?

      -Present on Admission

      -Not Present on Admission

      -Documentation Insufficient

      -Clinically Undetermined

      -Exempt from POA Reporting
