This value set includes community hospitals, tertiary referral hospitals, and acute care hospitals. Patients transferred to a skilled nursing facility are not excluded.

      For the STK-2 measure, there is a denominator exclusion that states "Inpatient hospitalizations for patients discharged to another hospital." Does discharge to a sub-acute care facility like a "Skilled Nursing Facility (SNF)" which will take over medication administration/management count as a denominator exclusion or not?
