      Thank you for your question. We agree that VTE-1 should be updated to only allow meaningful pairs of "medication" and "device" either during ED or during inpatient encounter. This change was made during the April 2014 measure annual update.

      Thank you for your question. We agree that VTE-1 should be updated to only allow meaningful pairs of "medication" and "device" either during ED or during inpatient encounter. This change was made during the April 2014 measure annual update.

      In the numerator logic in VTE1, if a patient comes in through the ED and is admitted as an inpatient within 1 hour, the combinations of "Medication, Administered not done: Medical Reason" and "Device, Applied not done: Medical Reason" allow for the following scenarios:

      1) "Medication, Administered not done: Medical Reason" w/in 1 day of inpatient admission,
      "Device, Applied not done: Medical Reason" in ED

      2) "Medication, Administered not done: Medical Reason" w/in 1 day of inpatient admission,
      "Device, Applied not done: Medical Reason" w/in 1 day of inpatient admission

      3) "Medication, Administered not done: Medical Reason" w/in 1 day of inpatient admission,
      "Device, Applied not done: Medical Reason" w/in 1 day after surgery that occurs w/in 1 day of inpatient admission

      4) "Medication, Administered not done: Medical Reason" in ED,
      "Device, Applied not done: Medical Reason" in ED

      5) "Medication, Administered not done: Medical Reason" in ED,
      "Device, Applied not done: Medical Reason" w/in 1 day of inpatient admission

      6) "Medication, Administered not done: Medical Reason" in ED,
      "Device, Applied not done: Medical Reason" w/in 1 day after surgery that occurs w/in 1 day of inpatient admission

      7) "Medication, Administered not done: Medical Reason" w/in 1 day of surgery that occurs w/in 1 day of inpatient admission,
      "Device, Applied not done: Medical Reason" in ED

      8) "Medication, Administered not done: Medical Reason" w/in 1 day of surgery that occurs w/in 1 day of inpatient admission,
      "Device, Applied not done: Medical Reason" w/in 1 day of inpatient admission

      9) "Medication, Administered not done: Medical Reason" w/in 1 day of surgery that occurs w/in 1 day of inpatient admission,
      "Device, Applied not done: Medical Reason" w/in 1 day after surgery that occurs w/in 1 day of inpatient admission

      Obviously if a patient does not come through the ED within 1 hour prior to inpatient admission, scenarios 1, 4, 5, 6 and 7 above would not apply, scenarios 2 and 9 are covered by the current logic, but scenarios 3 and 8 are not
