      ​Thank you for your inquiry regarding CMS68v13: Documentation of Current Medications in the Medical Record. This measure requires all eligible clinicians reporting on this measure to document a patient's current medications. MIPS eligibility is not limited to physicians and is inclusive of other clinical license types.

      We convene a measure expert work group (EWG) annually to review evidence for consideration in the Annual Update for the measure. For performance period 2023, the EWG has already met and did not recommend any changes regarding the removal of CPT code 99211 from the "Encounter to Document Medications" (2.16.840.1.113883.3.600.1.1834) value set. We appreciate your feedback and we will evaluate the appropriateness of removing this code with our EWG during the next Annual Update cycle.

    • Encounter code 99211 Pulls patients into the denominator who are not seen by an eligible clinician.

      when validating measure 68 for a not met status, we discovered that encounter codes 99211 are being pulled into the denominator for this eCQM.  These encounters can be nurse encounters where an eligible provider does not see the patient.  The measure calls for the eligible provider to document the list of medications.   In the CQM version of this measure, this encounter code is excluded.   See attached document.
