Need clarification for CMS 2

      ​​Thank you for your inquiry regarding CMS2v12: Preventive Care and Screening: Screening for Depression and Follow-Up Plan. Unlike CMS159, this measure is not prescriptive as to which standardized depression screening tool may be used so specific screening tool coding is not included in this measure. Instead, the measure guidance provides a limited list of examples of standardized depression screening tools that does include the Patient Health Questionnaire (PHQ-9) and PRIME MD-PHQ2. Therefore, per the measure specifications, the codes "Adolescent depression screening assessment" (LOINC Code 73831-0) and "Adult depression screening assessment" (LOINC code 73832-8) are used to indicate that depression screening assessment occurred on the date of or up to 14 days prior to the date of the qualifying encounter using any age-appropriate standardized, and validated depression screening tool developed for the patient population in which it is being utilized. This measure does not require documentation of a specific score, just whether results of the normalized and validated depression screening tool used are considered positive or negative. Since each standardized screening tool provides guidance on whether a particular score is considered positive for depression, the measure requires the result of the screening tool to be documented as negative or positive using the "Depression screening negative (finding)" SNOMEDCT code (428171000124102) or "Depression screening positive (finding)" SNOMEDCT code (428181000124104). We are not able to provide guidance on how you should or should not map assessments in your EHR and would suggest consulting with your clinical partners to determine an approach to ensure that the occurrence of the depression screening and its positive or negative result is captured in their EHR accurately.

    • Clients want our software when they fill out a PHQ-9 questionnaire to attach the appropriate LOINC codes to a PHQ-2 questionnaire. Can we do this?

      Clients want our software when they fill out a PHQ-9 questionnaire to attach the appropriate LOINC codes to a PHQ-2 questionnaire.  Can we do this?  

      Adult PHQ-2 LOINC Code = 73832-8
      Adult PHQ-9 LOINC Code = 44261-6

      put the 2 and 9 adult codes

      For ex. start with PHQ-9 adult, it generates a score, i then attach the neg or pos depression screening SNOMED to that questionnaire.  Can I use the same PHQ9 to qualify for CMS 2 if I attach the PHQ 2 LOINC codes as well?

      The transaction would look like:
      PHQ9 LOINC and score, and PHQ2 LOINC and SNOMED Negative or Positive from the same questionnaire.

      If that answer is yes,
      that same patient could qualify simultaneously for CMS 159 and CMS 2 even though one PHQ 9 questionnaire was administered?

      If that answer is yes as well,
      Why didn't CMS inclue PHQ 9 LOINCs in CMS 2 value set?
