​Thank you for your inquiry for CMS139v11 Falls: Screening for Future Fall Risk regarding the addition of "day of" to the numerator logic of the measure. The addition of "day of" to the measure logic was due to a timezone offset issue related to CQL logic structure. Adjusting the numerator logic to say 'during day of "Measurement Period"' will capture dates only from both relevant period and measurement period without time zone offset normalization, and thus all cases within the measurement period are captured and evaluated in the numerator as expected. Please note that this change does not imply that the fall screening has to be completed on the day of the encounter, just that it must occur during the measurement period.

If the wording change implies that fall screening has to be completed at the day of the encounter, we have a digital pre-checkin Patient Portal which currently includes a fall screening questionnaire and could be completed day(s) prior or the day of the encounter. If the Fall Screen has to be completed on the day of the encounter then we will have to remove that question from the questionnaire.
