      Hello and thank you for your inquiry about CMS108v10/VTE-1 Venous Thromboembolism Prophylaxis. There are two Heparin related value sets in VTE measures. Equivalent codes may be mapped to the value sets if necessary.

      The heparin codes provided in the attachment are included in the value set of Low Dose Unfractionated Heparin for VTE Prophylaxis (OID: 2.16.840.1.113762.1.4.1045.39). We hope this helps.

    • We are falling out of VTE 1 measure due to the fact that we mix our heparin in the hospital, we do not use pre-mixes. Several patients are missing the numerator even though they had heparin drips started within the allotted timeframe.

      We have several patients missing the numerator, however they were on heparin drips within the allotted timeframe, but being we don't use pre-mix heparin drips, we feel that this is the reason we are falling out.  I have worked with our vendor and my pharmacy team to attempt to identify which RXNorm we are using, no one was sure how to identify this, is there anyone that could assist in identifying the RXNorm we are using and then determine if we need to request our RXNorm to be added to the value set?
