      ​Thank you for your inquiry regarding CMS68v12: Documentation of Current Medications in the Medical Record. Patients receiving hospital care are not excluded from this measure. The "Encounter to Document Medications" (2.16.840.1.113883.3.600.1.1834) value set identifies qualifying encounters that would meet the denominator criteria. You can review codes included in each value set on the Value Set Authority Center (VSAC) website at https://vsac.nlm.nih.gov. The value set includes codes for care taking place in the hospital setting such as CPT codes 99221 – 99223.

      Measure 68 is supposed to be for an office visit but we getting penalized on our hospital visit. Are hospital visit are supposed to be excluded from this quality measure ?
