As per the measure guidance, only one glycemic event is counted towards the numerator per qualifying encounter in each measure regardless of the number of glycemic events that occur during that encounter.

      Although there are two separate glycemic events during each of your example encounters, the measure outcome for both cases is Numerator = 1, Denominator = 1, assuming other required numerator and denominator criteria for the measure are also met. We hope this helps.

      Can a patient be in the numerator multiple times during the same encounter for the same measure? 

      The question is ... can the patient be counted in the numerator for the same measure more than once for multiple episodes of hyperglycemia or hypoglycemia? 

      Example: 
      Patient #1 
      2/10/2023 
      BS 301 

      2/11/2023 
      BS 302 

      Is the numerator 2 (based on the number of high BS levels) and denominator 1 (Patient Encounter)? 

      OR once they have a BS higher than 300 once, is that counted only once in the numerator ?

      so that same patient 

      is the numerator 1 ( for high BS levels during encounter) and denominator 1 (Patient Encounter)?

       

       

      or

      Example: Does a blood sugar of 38 on Day 12 of admission and another blood sugar of 37 on Day 15 of the same admission count twice in the numerator for hypoglycemia?
