      ​​Thank you for your inquiry regarding CMS2: Preventive Care and Screening: Screening for Depression and Follow-Up Plan. Although we do not provide specific mapping guidance, review of the follow-up value sets found codes that would indicate crisis intervention or involve a care plan for a mental health crisis. Please review the specific codes included in the "Follow Up for Adolescent Depression" (2.16.840.1.113883.3.526.3.1569) and "Follow Up for Adult Depression" (2.16.840.1.113883.3.526.3.1568) value sets in the Value Set Authority Center (VSAC) to determine which is most appropriate for specific mapping of the Stanley-Brown Safety Plan/ Suicide Safety Plan.

      In addition to the Adult and Adolescent follow-up value sets, the "Referral for Adolescent Depression" (2.16.840.1.113883.3.526.3.1570), "Referral for Adult Depression" (2.16.840.1.113883.3.526.3.1571), "Adolescent Depression Medications" (2.16.840.1.113883.3.526.3.1567) and "Adult Depression Medications" (2.16.840.1.113883.3.526.3.1566) value sets identify appropriate follow-up for a positive depression screening.

      After reviewing post [CQM 5746|, we understand that additional assessments or screenings using a standardized tool will not qualify as a follow-up plan. Sanford Health would like guidance on mapping the Stanley-Brown Safety Plan/Suicide Safety Plan as an Intervention, Performed: Follow up for depression value set.  Eligible Clinicians utilize this intervention when self-harm is captured within screeners. We believe this plan qualifies and is not considered additional screening per the guidance on the eCQI resource.  We are seeking clarification on this intervention meeting measure intent for follow up when self harm is indicated in screenings.
