Denominator and Numerator Requirements for CMS139v11

      Thank you for your question regarding CMS139v11. This measure is looking for the percentage of every patient 65 years of age and older, who had a qualifying encounter visit during the measurement period and were screened for future fall risk at least once within the measurement period.

       

      The denominator criteria of CMS139v11 requires a qualifying encounter from any of the following value sets:

      "Office Visit" (2.16.840.1.113883.3.464.1003.101.12.1001)
      "Annual Wellness Visit" (2.16.840.1.113883.3.526.3.1240)
      "Preventive Care Services Established Office Visit, 18 and Up" (2.16.840.1.113883.3.464.1003.101.12.1025)
      "Preventive Care Services Initial Office Visit, 18 and Up" (2.16.840.1.113883.3.464.1003.101.12.1023)
      "Home Healthcare Services" (2.16.840.1.113883.3.464.1003.101.12.1016)

      "Preventive Care Services Individual Counseling" (2.16.840.1.113883.3.464.1003.101.12.1026)

      The numerator criteria of CMS139v11 requires a qualifying assessment from the “Falls Screening” (2.16.840.1.113883.3.464.1003.118.12.1028) value set.

       

      To further clarify, a fall risk screening must assess whether an individual has experienced a fall or problems with gait or balance. A specific screening tool is not required for this measure, however potential screening tools include the Morse Fall Scale and the timed Get-Up-And-Go test. The results of a fall risk screening assessment should be patient-reported. Clinically equivalent services may be mapped to the contents of the "Falls Screening" value set to satisfy the screening requirements of CMS139v11. If mapping is done, you should maintain documentation in case of a CMS audit. The date/time of the fall risk screening assessment should also be documented to meet screening requirements, i.e., at least once within the measurement period.       Hi,

      I am trying to figure out the requirements for the numerator and denominator for CMS eCQM ID: CMS139v11 (Quality ID: 318).  Requirements as in the denominator criteria for eligible cases and the numerator options for performance quality. I just need to know if certain billing or diagnosis requirements need to be met.
