A specific screening tool is not required for this measure, however potential screening tools include the Morse Fall Scale and the timed Get-Up-And-Go test. The results of a fall risk screening assessment should be patient-reported. Clinically equivalent services may be mapped to the contents of the "Falls Screening" value set to satisfy the screening requirements of CMS139v11. If mapping is done, you should maintain documentation in case of a CMS audit. The date/time of the fall risk screening assessment should also be documented to meet screening requirements, i.e., at least once within the measurement period.
