      Hi, I would like to know what are the outcome scores for ≥ the 40th percentile or quality performance scores ≥ the 10th percentile for DM, HTN and Depression measures. For example, for HTN, is 77.83% of outcome fall under the 40th percentile?

      We want to set our internal goal as an ACO and understand the thresholds for Diabetes, HTN, and Depression Screening. Thank you!
