      Thank you for your inquiry regarding CMS68v12: Documentation of Current Medications in the Medical Record. The "Encounter to Document Medications" (2.16.840.1.113883.3.600.1.1834) value set includes codes that identify appropriate encounters that would meet denominator criteria, including CPT codes 99221, 99222, and 99223.

       

      A denominator exception can only be taken if a provider documents a medical reason(s) for not documenting, updating, or reviewing the patient’s current medications list (e.g., patient is in an urgent or emergent medical situation where time is of the essence and to delay treatment would jeopardize the patient’s health status). A denominator exception cannot be taken for patients who are in a hospital that uses a different EMR system. The "Medical Reason" (2.16.840.1.113883.3.526.3.1007) value set includes codes that identify appropriate medical reasons that meet denominator exception criteria.

      For Measure CMS 68 - it includes cpt codes 99221, 99222, & 99223
      our providers see patients in the hospital and use a different EMT system. What is the protocol for our practice to not get penalized. Since we use different EMR systems and the document we can pull are summary of the visit of the patient the hospitals EMR system.
