Patients will meet criteria for denominator exclusions if any of the following are true: patient was discharged to another hospital, patient left against medical advice, patient expired, patient was discharged to home for hospice care, patient was discharged to a health care facility for hospice care, or patient has comfort measures documented.

      Based on the details provided in your scenario, your patients may qualify for the denominator exclusion criteria due to being discharged to a health care facility for hospice care. In order to meet this denominator exclusion criteria, patients must have the encounter discharge disposition SNOMEDCT contained in the value set "Discharged to Health Care Facility for Hospice Care" (OID: 2.16.840.1.113883.3.117.1.7.1.207), code 428371000124100 “Discharge to healthcare facility for hospice care (procedure)” or an equivalent code. Please consult your EHR vendor for mapping information if applicable.

    • Discharged disposition to skilled nursing facility on comfort measures

      We have a new EHR system (EPIC). We have patients that have been discharged to skilled nursing facilities on comfort measures.  The skilled nursing facilities are not hospice facilities but provide hospice care.  These patients are captured in the denominators.  How can we get these patients to pass the measure?
