    • Discharged disposition to skilled nursing facility on comfort measures

      We have a new EHR system (EPIC). We have patients that have been discharged to skilled nursing facilities on comfort measures.  The skilled nursing facilities are not hospice facilities but provide hospice care.  These patients are captured in the denominators.  How can we get these patients to pass the measure?
