      STK-5 captures ischemic stroke patients who are administered antithrombotic therapy, usually aspirin, starting the day of or day after hospital arrival. Not administering anti-thrombotic therapy after hospital arrival must be documented in the medical record by the MD/APN/PA or pharmacist and could be mapped to an appropriate code contained in the “Medical Reason” value set (2.16.840.1.113883.3.117.1.7.1.473). You can review the specific codes included in this value set on the Value Set Authority Center (VSAC) at https://vsac.nlm.nih.gov/. Please note STK-5 is time sensitive and that the reason must be documented in a discrete field and by the end of hospital day two in order to meet the denominator exception.
