The value sets used by the Eligible Hospital /Critical Access Hospital eCQMs can be downloaded from the eCQI resource center at https://ecqi.healthit.gov/eh-cah?qt-tabs_eh=1. Click on the ‘eCQM Resources’ tab. Then scroll down and click on ‘eCQM and Hybrid Measure Value Sets’. This will take you to the Value Set Authority Center (VSAC). Then click on the year you are interested in (e.g., “2023 Reporting/Performance Period of eCQM & Hybrid Measure Value Sets”). From there you are given many choices for your preferred sorting order, etcetera. You will need to login to VSAC before you can download a file containing the value sets. This file is zipped and you will need to extract the members by double clicking on the file and choosing ‘Extract’. Then you can filter on value set “Antithrombotic Therapy” (oid: 2.16.840.1.113762.1.4.1110.62) which includes both the anticoagulant and antiplatelet medication list.

Please note that this value set utilizes the semantic clinical drug (SCD) concept. Semantic Clinical Drug (SCD) codes are generalizable drug concepts, and each RxNorm clinical drug name reflects the active ingredients, strengths, and dose form comprising that drug. Further, these are restricted to generic, prescribable SCD codes.
You can find more information about SCD codes in the eCQM Logic and Implementation Guidance located here: https://ecqi.healthit.gov/sites/default/files/eCQM-Logic-and-Guidance-v6.pdf in the section titled “Drug Representations Used in Value Sets”. Please see below:

4.5 Drug Representations Used in Value Sets
Value sets referring to specific, non-vaccine, prescribable medications use generalized drug concepts (for example, RxNorm Semantic Clinical Drugs [SCDs]). It is expected that health IT vendors or providers will report the drug entities in patient data using the generalized drug concepts included in the defined value sets. These accord with CMS’s guidance about the preferred use of generalized drug concepts. eCQM Implementers should use the relationships found in RxNorm to support mapping between specific drug entities found in patient records to those found in the value sets provided…

Please note, per the measure guidance, only doses that are therapeutic for antithrombotic therapy should be mapped to lower dose formulations. To know which specific medications and dose forms are mapped to codes in this value set for your site, to meet measure intent, you will need to check with your informatics expert.
