CMS 145- Amount of encounters needed and are the encounters provider specific

      ​​Thank you for your inquiry regarding CMS145v11: Coronary Artery Disease (CAD): Beta-Blocker Therapy-Prior Myocardial Infarction (MI) or Left Ventricular Systolic Dysfunction (LVEF less than or equal to 40%). The encounters that qualify patients for the Initial Population of this measure do not have to be provider-specific, they can be from one or more providers.

      Depending on the timing of a patient's encounter and their CAD diagnosis and history of cardiac surgery, they may only need two encounters to meet the initial population. For example, if a patient has a Qualifying Encounter from the value sets in bold below (these value sets are included as both Qualifying Encounters and Qualifying CAD Encounters), a prior diagnosis of CAD without MI, and a history of Cardiac Surgery, the Qualifying Encounter will also count as a Qualifying CAD Encounter. However, if a patient is diagnosed with CAD without MI and has cardiac surgery after the first two (or more) Qualifying Encounters During the Measurement Period, the next encounter meeting the Qualifying CAD Encounter codes will count as the Qualifying CAD Encounter.
