      ​Thank you for your question regarding CMS139v11, Falls: Screening for Future Fall Risk. This measure is looking for the percentage of every patient 65 years of age and older who were screened for future fall risk during the measurement period. To further clarify, a fall risk screening must assess whether an individual has experienced a fall or problems with gait or balance. A specific screening tool is not required for this measure, however potential screening tools include the Morse Fall Scale and the timed Get-Up-And-Go test. The results of a fall risk screening assessment should be patient-reported. Clinically equivalent services may be mapped to the contents of the "Falls Screening" value set to satisfy the screening requirements of CMS139v11. If mapping is done, you should maintain documentation in case of a CMS audit. The date/time of the fall risk screening assessment should also be documented to meet screening requirements, i.e., at least once within the measurement period.

      Does every patient 65 and older required to fill out a fall risk questionnaire to satisfy the measure or only those that have had a recent fall?
