Thank you for your question. The CMS9v10 (ePC-05) logic will evaluate newborns with administered breast milk as identified in the Breast Milk value set (2.16.840.1.113883.3.117.1.7.1.30) without items listed within the "Dietary Intake Other than Breast Milk" value set (2.16.840.1.113883.3.117.1.7.1.27). "Dietary Intake Other than Breast Milk" value set (2.16.840.1.113883.3.117.1.7.1.27) includes water, formula, and glucose. Newborns with administered breast milk without other dietary intake will meet the numerator. However, for this example, the water is being used for irrigation of the orogastric tube and is part of the removal process and is not considered a feeding.

39 week gestation newborn being reviewed for PC-05, exclusive breast milk feeding.

Newborn intake and output flowsheet includes an orogastric tube placed on 11/11/22 at 1400; intake of 5mL of sterile water, followed by output of 27mL of air, 9mL of mucus and sterile water with removal of the orogastric tube on 11/11/22 at 1405.  All other newborn intake documented states breast milk feedings.

Would the sterile water be considered a medication as part of the procedure or considered a feeding?
