Per the measure specification, patients meet denominator exception criteria if the provider documents that the patient declined to participate as per the "Patient Declined" (2.16.840.1.113883.3.526.3.1582) value set or the "Medical Reason" (2.16.840.1.113883.3.526.3.1007) value set. You can review codes included in each value set on the Value Set Authority Center (VSAC) website at https://vsac.nlm.nih.gov.

      Specifically, the language included in the denominator exception, "Medical Reason(s) Documentation of medical reason for not screening patient for depression (e.g., cognitive, functional, or motivational limitations that may impact accuracy of results; patient is in an urgent or emergent situation where time is of the essence and to delay treatment would jeopardize the patient's health status)" listed in the CMS2v11 measure specification provides only examples. The denominator exception can be applied when the eligible clinician determines the screening is contraindicated or not indicated per the "Medical Reason" (2.16.840.1.113883.3.526.3.1007) value set.

      Inquiry: For situations other than medical in which lists "unable to screen" (provider is unable to screen due to) reasons such as lack of interpretation services, panic attacks, or severe mental trauma, should these be listed as exclusions or exceptions?
