      ​Thank you for your inquiry. ​In order for the patient to qualify for the denominator exclusion, you will have use the codes in the "Comfort Measures" (OID 1.3.6.1.4.1.33895.1.3.0.45). This can be done either using the codes directly or mapping the appropriate codes in your EHR to the "Comfort Measures". Technically, “Comfort Care” and “End of Life” are both acceptable terms for CMO. Please make sure that there is proper documentation in you decide to map your codes to the value set.

      In reviewing the orders placed on the day of and day after admission/ICU admission, I noticed that the Inpatient Comfort Care End of Life Order Set was used.  If an order is placed for something on this order set (such as pain medication),  which is linked to the IP Comfort Care/End of Life Order Set (because the pain med order was selected from the order set), would this suffice to conclude this case as  "Comfort Care Intervention" to exclude the case if we were to map the order set code to an appropriate SNOMED Code? Or does there need to be a specific order placed that the patient is admitted for comfort care/end of life care?
