Per the measure specification, codes 372067001, 385721005, 385724002, 385725001, and 385726000 are included in the "Follow Up for Adolescent Depression" (2.16.840.1.113883.3.526.3.1569) and "Follow Up for Adult Depression" (2.16.840.1.113883.3.526.3.1568) value sets that identify acceptable follow-up to meet numerator criteria. Therefore, these codes can be used to document follow-up for a positive depression screen.       Approximately 30% of patients visits to primary care are for mental health problems and the number of mental health patients treated solely in primary care may be as high as 70%. Primary Care Providers have the skill set to provide care, education and counseling for mild to moderate depression episodes. This could help reduce strain on an already severely over-burdened mental health specialty.

      I am referencing Jira ticket CQM-5687, in which the solution states that a PCP cannot document "patient to return when and if necessary" to the PCP clinic, as it is considered a self-referral and does not meet the intent of the measure.

      Would the following meet the intent of follow-up for a positive depression screen? 

      The patient has a positive depression screening during a primary care visit and the primary care practitioner provides and documents one of the following: code 372067001 "Implementation of measures to provide psychological support", 385721005/385724002 "Coping support assessment/management", or 385725001/385726000 "Emotional support assessment/education".
