      If a patient is admitted with history of DVT and Has factor V Leiden with previous failed XARELTO and Warfarin series and have been on ELIQUIS
      will this meet the measure for eCQM intervention for VTE 1 and VTE 2?

      If a patient has been on ELIQUIS for Factor V Leiden will this meet the measure for VTE 1 and VTE 2
