Thank you for your inquiry on CMS996, Appropriate Treatment for ST-Segment Elevation Myocardial Infarction (STEMI) Patients in the Emergency Department (ED). The intent of the timing for the denominator exclusions in the logic “overlap of DxExclusion.prevalencePeriod overlaps Interval[Start of EDwSTEMI.relevantPeriod - 90 days, start of the EDwStEMI.relevantPeriod])” is for the diagnosis (namely stroke, facial/head trauma or peptic ulcer) to start within 90 days before the start of the ED encounter or at the start of the ED encounter. The diagnosis abatement does not need to overlap with the 90 days prior to or the start of the ED encounter. We will consider the logic and alignment with the measure intent during the next Annual Update.

    • Unable to implement logic without this clarification

The group of Diagnosis for Denominator Exclusions called "Active Exclusion Diagnosis Within 90 Days Before Start of ED Encounter" has confusing logic.  What is intent of timing for overlap of DxExclusion.prevalencePeriod overlaps Interval[Start of EDwSTEMI.relevantPeriod - 90 days, start of the EDwStEMI.relevantPeriod])?

Does this mean the diagnosis has to start within 90 days before the Encounter Start AND the Diagnosis onset to abatement has to overlap the Encounter Start?

      OR

Does this mean the diagnosis Start has to overlap the period that starts 90 days before the Encounter Start?

The text description seems to be looking for a diagnosis that starts "within" the 90 days before ED Encoutner Start. But, the way the logic is written is a bit ambiguous.

      Please clarify.

      Thank you, Meg
