Thank you for your inquiry regarding 108v11: Venous Thromboembolism Prophylaxis. Yes, you can map the retired codes to the more specific codes in your examples for Obstetrics and VTE. The original concept which you were using, and is now deleted, combined the two separate concepts into one.

      For your examples regarding Diagnosis:
      - for VTE it would depend on what is actually occurring with the patient and if it is phlebitis or thrombophlebitis.
-for obstetrics, if it is a delivery episode, it would be a childbirth code and if postpartum, it would be puerperium.

On this CQM-5336 the Mathematica EH eCQM Team said this:

We have this case on this Value set (also we have another cases). But I'm going to use this first.

      Example 1:

      • Value Set Diagnosis: Venous Thromboembolism using "Venous Thromboembolism (2.16.840.1.113883.3.117.1.7.1.279)"
        • This code are deleted on 2023 on this VS:
266267005 - Deep vein phlebitis and thrombophlebitis of the leg (disorder)    SNOMEDCT

      • We see I think we can mapping this two codes to the deleted code (266267005) :
        • 25114006 - Phlebitis of deep veins of lower extremity (disorder) SNOMEDCT
        • 40198004 - Thrombophlebitis of deep veins of lower extremity (disorder) SNOMEDCT

      Example 2:

On this Value set this code are deleted on 2023:

valueset "Obstetrics" (2.16.840.1.113883.3.117.1.7.1.263) 200224003 Superficial thrombophlebitis in pregnancy and the puerperium - delivered (disorder) SNOMED CT

      • We see I think we can mapping this two codes to the deleted code (200224003):
        • 308136002 - Superficial thrombophlebitis in pregnancy (disorder)
        • 308137006 - Superficial thrombophlebitis in puerperium (disorder)

My question is we can do this mapping one code to two codes (1 to 2)? What of these 2 codes can be the principal diagnosis, if we have the case that the patient have the 2022 code on principal?

For you're information we are vendor of EHR. I don't work in a Hospital, the question are because we are programming this on our EHR. The Hospital don't mapping these codes because don't have access to programming our EHR.

      Thanks
