The intent of these value sets is to identify people whose titer count is high enough that the person is immune to the disease in question. Titer test results can be reported as either qualitative or quantitative results. Therefore our value sets include LOINC codes for titer tests rather than units/volume or just the presence of antibodies. We also try to include all tests that are pertinent, not just the most commonly done tests. That being said, we are constantly refining the lists to make sure the most applicable codes are included. We will consider your comments and any revisions to the lists will be made at the next annual update.

Re: Question about a few antigen value sets of MU1 EP Measure NQF0038

      To Whom It May Concern,
Recently we were doing some comparisons between the LOINC terms of Value Set Authority Center (VSAC) used in the MU Quality Measures and our set of common LOINC result codes. One thing we noticed was that the MU1 EP Measure NQF0038 for some antigen tests just included the titer LOINC terms. In contrast, our LOINC Top 2000 list mostly just included the tests reported as Pos/Neg (they have the Property/Scale pattern of ACnc/Ord in LOINC).
      For example,
The LOINC terms of MU1 EP Measure NQF0038 for Measles Antigen Test (13 codes), Varicella Zoster Antigen Test (15 codes) and Hepatitis B Antigen Test (2 codes) were all for titer measurements. But the test for these 3 categories in Top2000 are all reported as units/volume (Measles: 12232-5, 5244-9, 20479-2, 35275-7; Varicella: 5403-1, 19162-7, 15410-4, 8047-3, 5404-9).
For the Mumps antigen test, MU1 EP Measure NQF0038 included 17 titer and 2 units/volume LOINC terms. But our top2000 list only included the terms for presence or units/volume measurements (25418-5, 22415-4, 6476-6, 7966-5).
All of the 9 LOINC terms in MU1 EP Measure NQF0038 for the Rubella Antigen Test were for titer measurement. As for the Top2000 list, it contains 7 LOINC terms for units/volume measurement (22496-4, 5332-2, 5334-8, 8014-3, 20458-6, 5335-5, 8015-0).
We appreciate if you could provide some explanation for your approach/logic used in selecting the LOINC terms for VSAC value set.

      Sincerely,
