CMS104v10 Q-STK-2: Discharged on Antithrombotic Therapy

      ​Currently there are no generic prescribable RxNorm codes for 81 mg aspirin oral/enteric tablet or for the dosing form of enoxaparin 120mg/0.8mL medications. The medication value sets developed for the program contain RxNorm codes that are Semantic Clinical Drugs (SCD), generic, and prescribable. Currently these are the only SCD RxNorm code available for 81 mg aspirin. Semantic Clinical Drug (SCD) codes and are generalizable drug concepts. Each RxNorm clinical drug name reflects the active ingredients, strengths, and dose form comprising that drug. Your local medication codes need to be mapped to the SCD RxNorm codes found in the value set in order to satisfy the electronic clinical quality measure. If there are relevant updates to RXNORM we will consider adjustments to the value sets for the next Annual Update.

      The intent and purpose of a value set helps to decide whether a mapping makes sense. Valueset Antithrombotic Therapy Antiplatelet OID: 2.16.840.1.113762.1.4.1110.61 intent is described by its Purpose (see below):

      This is an RxNorm valueset and includes oral, rectal and injectable dose forms.
           Purpose:
                Clinical Focus: The purpose of this value set is to represent concepts of medications that are antiplatelet medications used to reduce stroke mortality and morbidity.
                Data Element Scope: This value set may use a model element related to Medication.
                Inclusion Criteria: Includes concepts that represent a medication that is an antithrombotic medication, including oral, rectal, and injectable dose forms.
                Exclusion Criteria: Excludes concepts that represent enoxaparin and heparin generally given for VTE prophylaxis.

      Has there been any consideration of including the oral/enteric coated form of ASA 81mg and the dosing form of enoxaparin 120mg/0.8mL medications in future versions of the specification? If not, what is the rationale and why was it removed? Can numerator compliance for the measure be credited in use of these medications? We appreciate any clarification. Thank you.
