      Thank you for your inquiry for the eCQM measure CMS138v11, Preventive Care and Screening: Tobacco Use: Screening and Cessation Intervention. We assume that this inquiry is about the performance period 2023 specification. The measure currently does not distinguish between the different types of tobacco smoking, however, a documentation of the screening using any of the codes in the “Tobacco Use Screening” value set is required to meet the screening criteria. Specifically, this measure requires a tobacco screening via QDM datatype "Assessment, Performed" along with the value set "Tobacco Use Screening", and requires a result using the QDM attribute "result". The "result" attribute must be identified as either "Tobacco Non-User" or "Tobacco User" in order to meet measure criteria. If this terminology is not natively supported by your EHR, you may map clinically equivalent services to the codes contained in the above mentioned value sets. Please note, if mapping does occur, you should maintain documentation in case of a CMS audit.

      If we have documentation of the patient as "never smoked" or "former smoker" but does not have smokeless screening completed, will this patient still meet the tobacco screening measure? Or should the patient be screened both for smoking and smokeless to meet the measure? 

      Please see the attached example. If we do not have smokeless information but pt's smoking information documented, we want to make sure we can count this patient as "met" for this measure.
