The measure stewards have determined that allergy criteria as denominator exceptions are not always appropriate, since an allergy to a single medication will cause the patient to be excluded. In the case of very broad value sets, that include numerous “types” of medications, being allergic to a particular one does not justify the exclusion. Rather, in these situations, the patient would have to be allergic to all medications in the value set in order to be excluded. The representation of such a criterion is, of course, not practical, but it should be very rare (if not impossible) for a patient to be allergic to all anticoagulants and all antithrombotics, for example. As such, during the April release, allergy criteria were removed from the denominator exceptions for the following measures: STK-3 (allergy to anticoagulants) and STK-2 (allergy to antithrombotics).

      We are seeing some inconsistencies regarding Allergy exceptions in the published release notes, from the April 1 update and are looking for clarification/direction.
      Example 1: STK-3, Anticoagulation Therapy for Atrial Fibrillation/Flutter, Medication, Allergy: Anticoagulant Therapy, has been removed under denominator exceptions.
      Example 2: STK-6, Discharged on Statin Medication, the allergy exception still exists, however a new value set is referenced - Statin Allergen.
      Example 2 appears preferred, as it allows mapping to the Allergy response, in the Medical reason value set and support clinical workflow. However without a "medication allergy" value set (as in STK-3), that mapping cannot occur.
      Please advise.
