The intent of the measures is to exclude women who had a bilateral mastectomy or who have a history of a bilateral mastectomy or for whom there is evidence of a right and a left unilateral mastectomy. A unilateral mastectomy with unspecified laterality is allowed in the measure logic given that the anatomical location is either right or left using SNOMED right/left qualifier value codes. The codes that are listed in the attachment that do not specify laterality, may be mapped using the value set "Unilateral Mastectomy, Unspecified Laterality" (2.16.840.1.113883.3.464.1003.198.12.1071). Please note if mapping is done, you should maintain documentation in case of a CMS audit.
