      ​Thank you for your clarification request for CMS68v11: Documentation of Current Medications in the Medical Record. CMS68 is an episode-based measure of the percentage of visits for patients aged 18 years and older for which the MIPS eligible clinician attests to documenting a list of current medications. Attest is the operative word of the measure intent. Only a MIPS eligible clinician can attest to the documentation of medications, but they do not necessarily need to be the clinician who documents current medications during the encounter. The list of MIPS eligible clinicians can be found on the CMS Quality Payment Program website.

      For 2022 eCQM submission period, our vendor is stating that Eligible professional or eligible clinician is MD, DO, NP, and PA Providers (Physicians or advanced Practitioner) , does not include RN or LPN or MA to qualify as numerator in this measure.

       

      Per Spec: 

      Numerator: Eligible professional or eligible clinician attests to documenting, updating, or reviewing the patient's current medications using all immediate resources available on the date of the encounter

       

      Is the intent for the Numerator to be only a Provider/Practitioner as MD, DO, NP or PA to do the Medication Reconciliation or can it also be a Nurse as RN, LPN, or Medical Assistant as MA doing a medication History prior to them seeing the provider? What qualifies the patient to have an Met, Done outcome  in this measure CMS68 (Documentation of Current Medications in the Medical Record) ?
