      Thank you for your inquiry regarding CMS69v10: Preventive Care and Screening: Body Mass Index (BMI) Screening and Follow-Up Plan. As a patient-based measure, the measure is to be submitted a minimum of once per performance period for patients seen during the performance period. If a patient sees multiple providers with a shared EHR, each provider who had a denominator eligible encounter could report the measure for the performance period. To meet numerator criteria, the patient must have the numerator quality actions performed within the specified timeframe and documented in the EHR. The measure does not require that the clinician who is reporting the measure performs the actions to receive numerator credit. In the scenario provided, both providers A and B would receive numerator credit provided the BMI and the follow-up plan that were documented both met all numerator criteria.

      Here is the scenario we are questioning for EC measure CMS69v10

      Multiple Providers in one single database.  Provider A has a visit with the patient on 6/1/2022 and charts a BMI value but no follow-up, then Provider B sees the patient on 7/1/2022 and charts a follow up for BMI.

      1. Does Provider A get credit for the follow-up being performed, if reporting as an individual, since the follow up action was charted on/after the 6/1/22 visit in the database even though it was by another provider?

      2. Does Provider B get credit for the BMI value that was charted 6/1/22, when reporting as an individual, because an eligible professional or their staff measured both height and weight within the 12 months prior to the visit on 7/1/22 in the same database?
