      ​Thank you for your comment about the eSTK-5 measure and valueset “Antithrombotic Therapy” (2.16.840.1.113762.1.4.1110.62). You can map "Heparin 20,000 units in dextrose 5% in water 500 mL Premix" to one of the heparin codes that you listed to satisfy the electronic clinical quality measure.

      The medication value sets developed for the program contain RxNorm codes that are Semantic Clinical Drugs (SCD), generic, and prescribable. Semantic Clinical Drug (SCD) codes and are generalizeable drug concepts. Each RxNorm clinical drug name reflects the active ingredients, strengths, and dose form comprising that drug.

      You can find more information about this in the eCQM Logic and Implementation Guidance located here: https://ecqi.healthit.gov/sites/default/files/eCQM-Logic-and-Guidance-v5.pdf in the section titled "Drug Representations Used in Value Sets". If there are relevant updates to RXNORM we will consider adjustments to the value sets for the next Annual Update.

    • Seeking to understand why heparin drip is not included in the antithrombotic therapy value set

      Curious as to why the following medication is not included in the CMS72v10 antithrombotic therapy value set:

      Heparin 20,000 units in dextrose 5% in water 500 mL Premix

      We do see the following heparin drips under the value set and wondered whether it might be the dextrose or the concentration of heparin that resulted in it not being included:
