      Thank you for your question. The encounter encompasses the entire period from presentation to the ED/direct admit through discharge. The “Clinical Quality eMeasure Logic and Implementation Guidance v1.5” document provides the definition of Episode of Care. Additional guidance can be found in the document under section ‘6.1.28 Measure 188v2’ regarding the determination of arrival time.

      During the 2014 annual update period, the logic was updated to incorporate the ICU/Non-ICU pathways.

      The Data Dictionary abstraction guidelines for the legacy measure decision point "Compromised" states:

      "f there is physician/APN/PA documentation within 24 hours after hospital arrival that the patient has a “prolonged” QT interval (QTc), select value “1”."

      Since this is looking for 24 hours after hospital ARRIVAL, the logic statement in the CQM is not sufficient. The current logic is:

      OR: "Diagnosis, Active: Hospital Measures-Prolonged QT Interval" <= 24 hour(s) starts after start of "Occurrence A of Encounter, Performed: Encounter Inpatient"

      This is only looking for 24 hours after admission, if the patient had an ED encounter prior to his inpatient admission, the time frame would need to be based on the arrival to the ED.
