      ​​Thank you for your question about the 71/STK-3 measure. The medication value sets developed for the program contain RxNORM codes that are Semantic Clinical Drugs (SCD), generic, and prescribable. There is no semantic clinical drug (SCD) concept for Lovenox 90 mg currently. If there are relevant updates to RxNORM we will consider adjustments to the value sets for the next Annual Update. In the meantime, your local medication codes need to be mapped to the SCD RxNORM codes found in the "Anticoagulant Therapy" (2.16.840.1.113883.3.117.1.7.1.200) value set in order to satisfy the electronic clinical quality measure.

    • enoxaparin 90 mg is not recognized as appropriate anticoagulant therapy at discharge (STK 3)

      If the patient is discharged home with Lovenox 90 mg (0.9ml SQ injection every 12 hours) and still did not pass the measure.  the value set includes .8 ML enoxaparin sodium 100 mg/mg prefilled syringe, .8 ML enoxaparin sodium 150 mg/ml prefilled syringe, 1 ML enoxaparin sodium 100 mg/ml prefilled syringe, 1 ML enoxaparin sodium 150 mg/ml prefilled syringe and enoxaparin sodium 100 mg/ml injectable solution.  Should the dosage prescribed in my example be counting in the numerator?  why is not passing the measure?
