The CMS Blueprint includes recommended vocabulary standards for eMeasures. We are following the recommended standards from CMS.
Quality Data Model Category or Clinical Concept: Intervention
(Note: Intervention is being retired—it is incorporated under Procedure)
Quality Data Model Type (State) Clinical: Intervention
      (Ordered, Performed, Recommended, Declined)
      Vocabulary Standards: SNOMED CT
      Transition Vocabulary: CPT, HCPCS,
      ICD-9-CM Procedures,
      ICD-10-PCS

The eCQMs should moving forward improve alignment with the recommended code systems--it is expected systems would remap these older concepts if still used in the system. Thank you for your feedback and CMS and ONC will continue to request feedback on the recommended code systems going forward.

In the Stage I version of this measure, the patient could be counted as having received counseling for physical activity or counseling for nutrition if the provider recorded diagnosis ICD-9 V-codes, specifically V65.41 and V65.3. These are widely used codes that providers use to represent the provision of these services. Note that both of these codes are still used to define the numerator population for measure CMS 69/NQF 421. But these codes have been removed from the value set for counseling for physical activity and counseling for nutrition without explanation. Please restore these codes as this represents a burden for many providers.
