Question about exclusion based on discharge to another inpatient care facility. How does this exclusion apply to discharges an inpatient care facility in which the patient is receiving short term rehab (i.e. Swing Bed admission to CAH)?

The measure excludes patients discharged to an inpatient facility, identified by the value set "Discharge To Acute Care Facility" (2.16.840.1.113883.3.117.1.7.1.87). This value set includes codes for three facility types - community hospitals, tertiary referral hospitals, and acute care hospitals. Patients discharged to a critical access hospital that can be mapped to one of these three facility types should be excluded from the measure.

      We serve a rural health care network in which non-acute care can be provided by CAHs in Swing Beds. Patient's can transfer from an IP admission to a non-acute swing bed admission within the same network. In effect the patient is admitted to a facility but not technically receiving inpatient or acute services. Are discharges to swing beds excluded or not excluded. Do we consider the facility or the discharge status? Thank you.
