      History​ of Blood Clots (Z86.718) does NOT satisfy the measure because it is not included in the Venous Thromboembolism value set (2.16.840.1.113883.3.117.1.7.1.279). The numerator logic identifies history by looking for Venous Thromboembolism diagnoses encoded in SNOMEDCT or ICD10CM that started before the encounter, and uses diagnosis prevalence period to indicate if the timing of the diagnosis meets measure criteria. Z86.718 is a history situational status code that is used for denoting personal history of a condition. Unfortunately, using the suggested Z code is not feasible for eCQMs and there is no corresponding SNOMED code that could be mapped to align​ with the measure intent.
