      Thank you for your inquiry regarding Quality ID#130: Documentation of Current Medications in the Medical Record. The guidance for this measure states that “eligible clinicians reporting this measure may document medication information received from the patient, authorized representative(s), caregiver(s) or other available healthcare resources”. The process used by your nursing facilities of “uploading the Transfer orders, scanning them into our documents in our own EHR, documenting in our visit notes that the medications were reviewed and reconciled in the facility EHR at every visit” falls within the “other available resources” criteria in the measure guidance, and therefore, would satisfy the requirement for eligible encounters.

      Our group of Providers work in several different Nursing Facilities. We have our own EHR and complete our notes this way. Our MA's check the different facilities each morning for new admission and retrieve a copy of all admission documents from the Facilities EHR and upload them to our EHR. The Medications are only entered into the facilities EHR by the Facility staff, we dont enter them into ours, we just make sure we have the list scanned into our EHR, and make any adjustments to those medications in our visit notes, and document in our note "Medications were reviewed and reconciled in the facility EHR". My Question is: By uploading the Transfer orders, scanning them into our documents in our own EHR, documenting in our visit notes that the medications were reviewed and reconciled in the facility EHR at every visit- will this meet the specifications of this measure enough for our Providers to click the Performance Met option "Eligible clinician attests to documenting in the medical record they obtained, updated, or reviewed the patients current medication" ?
