Logic to determine ICU Admission or Transfer inconsistant between VTE and PN

      Thank you for your question. Currently, EHR Vendors or Hospitals can use ICU location or other meaningful approaches to identify an ICU admission or transfer encounter event. In another word, the criterion can be translated to: "check if an ICU admission or transfer encounter occurred during a regular inpatient encounter using any means that are suitable for an EHR or hospital including but not limited to using ICU location.” Going forward, the eSpecifications will be standardized to capture ICU encounter in a consistent approach across eCQMs whenever possible.

      The Pneumonia CQM determines if a patient was an ICU admission or transfer based on if the have an ICU Location during an inpatient encounter, for example:

      AND: "Occurrence A of Encounter, Performed: Hospital Measures - Encounter ED (facility location arrival datetime)" <= 24 hour(s) starts before start of "Occurrence A of Encounter, Performed: Encounter Inpatient (facility location arrival datetime, facility location: 'Hospital Measures-ICU Locations')"

      VTE on the other hand has completely different logic to detemine if the patient was an ICU Admission or transfer:
      ◦AND: "Occurrence A of Encounter, Performed: ICU Admission or Transfer" during "Occurrence A of Encounter, Performed: Inpatient Encounter"

      We question the validity of the method being used in VTE. We believe it is more likely that a hospital would track ICU usage by location during an encounter, not as another instance of an Encounter, Performed?
