      Thank you for your inquiry. Patients with a Principal Procedure (denoted by rank=1 in the logic) of SCIP VTE Selected Surgery will be excluded from the denominator. The logic ensures that the SCIP VTE selected surgery occurred during this inpatient encounter by looking at surgery start/stop time. If the documentation provided (“the patient was directly admitted for surgery”) indicates that this patient is admitted to the hospital for an elective surgery, the inpatient admission datetime should be same as (or before) surgery start time, and the patient should be excluded.

      I am trying to confirm that cases where a SCIP VTE Selected Surgery are being handled correctly for VTE-1.  We are seeing cases where the patient was directly admitted for surgery but the inpatient status does not start until after the surgery.  These cases do not have an outcome of denominator exclusion per our vendor logic. Is the intent of the measure that the surgery would only count as an exclusion if it occurred after a patient was considered an inpatient? In looking at the 2021 eCQM Flow for this measure I see under the list of value sets for the surgeries that "Rank=1 during encounter" is listed.  Is that what indicates it is only an exclusion if the surgery was done after the patient became an inpatient?
