      ​​Thank you for your inquiry regarding CMS2v11: Preventive Care and Screening: Screening for Depression and Follow-Up Plan.

      Per the measure specification, "This eCQM is a patient-based measure. Depression screening is required once per measurement period, not at all encounters." This means that the depression screening (and follow up if applicable) are required a minimum of once per patient during the measurement period. However, if multiple screenings occur during the measurement period, the most recent screening will be used to evaluate the measure.

      The measure specification clarifies that documentation of a specific score from a depression screening instrument is NOT required. A clinician should refer to the instructions for the standardized tool used to screen for depression to interpret whether a patient's score is considered positive or negative for depression. Clinicians may then determine how the specific score should be documented and then mapped in their EHR in order to report the screening result as negative or positive for the measure.

    • Need an understanding of the frequency of screening during measurement year.

      According to the 2022 measure spec, this measure is a patient-based measure. The guidance section states that screening is required once per measurement year, not at all encounters. Can you please clarify/confirm that this does not have to be done at EVERY patient encounter. 

      Also, does the provider have to discreetly document their interpretation of the score (whether score is positive/negative)? Or does the tool itself determine what scores are considered positive or negative? 

      The measure spec specifies that the standardized screening tool provides guidance on whether a particular score is considered positive.
