Thank you for your question about CMS108v10 and CMS190v10, VTE measures. The VTE measures look for medication administered during a qualifying timeframe, which is the day of or day after admission or procedure. An Oral Factor Xa Inhibitor prescribed during the qualifying timeframe qualifies for the numerator regardless of whether the patient was admitted with an existing prescription, assuming other conditions are met. Oral Factor Xa Inhibitors administered during the qualifying timeframe meet numerator criteria if patient has an atrial fibrillation or VTE diagnosis or a hip/knee replacement surgery. Medication administrations should be recorded in the EHR.

Patient may be admitted with a home medication of Oral Factor Xa Inhibitor. Therefore, the provider continues Oral Factor Xa Inhibitor as an inpatient medication as a prophylaxis. Has there been any consideration to evaluating records with a home medication of Oral Factor Xa Inhibitor that also have it administered during the algorithms specified timeframe as an appropriate prophylaxis?
